• Compensation: - $141.42 - $150.00/hr- $10,000 Sign-on Bonus & Relocation package available (Will be prorated for PT)- Productivity Bonus as well (Will be prorated for PT)

Initial assessment and patients will then be passed onto the LCSW team, next visits will be for follow-up and prescription refills. Truly integrated healthcare approachQUALIFICATIONS: - CA license REQUIRED- 1-year experience in an ambulatory care setting REQUIRED- Experience in FQHC PREFERRED- Bilingual (SPAN-ENG) PREFERRED
