JOB TITLE: TENANCY SUPPORT SPECIALIST

Areas of Responsibility: The Tenant Support Specialist (TSS) addresses the housing needs of adults who are disabled, chronically homeless, and seeking permanent housing. Services include the assessment, identification, arrangement and provision of pre-tenancy, housing stabilization, tenancy sustaining, and housing-focused service coordination. The TSS works collaboratively with social services providers in the community to identify and address barriers to better quality of life for patients who are currently unhoused.

 

Essential Duties and Responsibilities and Expectations:

 

·        Develops, implements, monitors, and evaluates individual patient care plans based on social needs. Triages non-medical needs related to, housing status, food security, transportation, physical safety, social support of client and other barriers related to health and well-being

 

·        Conduct outreach to housing-insecure individuals with the goal of connecting them to a more comprehensive, holistic, integrated healthcare system. Create a patient centered case plan focused around all social determinants of health.

 

·        Provides home visits to recently housed people in support of their self-determined housing stability plan.

 

·        Plans and implements social events, peer support groups, and community volunteer opportunities for newly housed tenants.

 

·        Identify available community resources for specific patient populations and act as a liaison between client and other community organizations. Makes appropriate patient referrals to community resources and organizations.

 

·        Actively participate in and coordinate with community housing providers and initiatives, such as Housing First, Coordinated Entry Systems, Frequent User System Engagement (FUSE), etc.

 

·        Facilitate the entire housing process, including assistance with housing search, housing applications, housing voucher applications, housing orientations, inspections, lease renewals, the move-in process, the move-out process, and housing relocation (when applicable).

 

·        Reviews current housing assessment (HMIS) scores and matches clients to housing type via Coordinated Entry System protocols.

 

·        Serve as a liaison between landlords, property owners, clients, and agencies providing ongoing supportive services to mitigate conflict and support retention of tenancy.

 

·        Provides education to clients, staff and community members. Create materials as needed.

 

 

 

 

·        Provide advocacy from a client’s perspective in program decision making and planning as it relates to housing.

 

·        Network and collaborate with area Housing Resources and maintains a presence at all meetings.

 

·        Coordinate with other housing focused organizations, groups and staff in the area including government agencies, jails, pre-release centers, sober living, etc.

 

·        Maintain Client Records – a) Maintains accurate documentation of service objectives and outcomes as well as other services in accordance with Federal, State, County and grant guidelines, b) Maintains client related data systems, including case notes and HMIS entries; c) Maintains a Property Manager/Owner Contact Log to meet set expectations, d) Prepares related reports including but not limited to: outcomes, and successes.

 

Minimum Qualifications (Experience/Education):

·        Bachelor’s degree in social-service related field required. Relevant experience may be substituted for education.

·        At least one year experience interacting with individuals who have mental health or co-occurring mental health and substance use disorders required.

·        Familiarity with Federal and local housing programs.

·        Knowledge of housing resources, subsidy programs, and the Continuum of Care for homeless persons.

·        Communication and teamwork: strong communication skills and patience.

·        Self motivation and development.

·        Efficiency and effectiveness: plan, organize, prioritize, and coordinate work flow effectively, work independently and meet multiple deadline, attention to detail and accuracy.

·        Adaptability to change and problem solving skills.

·        Data collection.

·        Ability to deal with both concrete and abstract situations.

·        High ethical standards, values and good judgment.

·        Strong commitment to team work, transparency and respect to diversity.

·        Valid Driver’s License and must be able to obtain MT Driver’s License within 6 months of hire.

·        Meet background check standards with a history and driving record acceptable to agency insurance carrier.

 

 

Employment Status: Full Time Non-Exempt Permanent, 40 hours/week

Salary Range: $22-26 per hour
