Targeted Case Manager

Provides case management services and assists an individual in gaining access to needed medical, social, educational, and/or other services. Must be able to provide a culturally appropriate assessment, and review or complete a plan of care on a periodic basis. Maintain strict confidentiality at all times!

Requirements

Bachelor's degree in social work, sociology, psychology or counseling from an accredited university or college AND two years of case management experience in social service, or health-related fields. Experience with data gathering and reporting, and conducting assessments. Submit and pass both a criminal background check with Federal, Tribal and State Law Enforcement and pre employment alcohol and drug screening.
