Signify Health is seeking a per diem In-Home Nurse Practitioner or Physician Assistant to deliver high-quality, value-based care to patients in their homes. In this flexible, field-based role, you will perform comprehensive health evaluations, review medical histories and medications, identify gaps in care, and develop clear, patient-centered care recommendations. Using Signify Health’s technology and analytics, you’ll document findings in the EMR, support care coordination with health plans and providers, and contribute to improved outcomes for vulnerable populations. You’ll work independently in the community while collaborating virtually with an inclusive, mission-driven clinical team focused on innovation and better healthcare experiences.
