• Shift: Days, 5x8s (8:00 AM – 4:30 PM)

Requirements

• Active California RN License (pending license accepted if active by start)
• Minimum 2 years of recent Acute Care Hospital Inpatient Case Management experience
• Current experience in Care Coordination and Admission Reviews within the last 6 months
• Epic experience required
• InterQual knowledge required
• Previous travel experience required (first-time travelers not accepted)

Responsibilities

• Coordinate patient care from admission through discharge
• Complete admission reviews using InterQual criteria
• Develop discharge plans and coordinate post-acute services
• Arrange DME, SNF placement, transportation, and community resources
• Collaborate with physicians, nursing, social services, and interdisciplinary teams
• Ensure appropriate utilization of healthcare resources while advocating for patients
