Job Description
Title
LNA - Hospice

Schedule
8-hour night shifts. May work part or all of every weekend. Must be flexible with shift hours.

Assignment Details
Contract Length: 6 weeks
Guaranteed Hours: 40 hours per week

Requirements
- Valid VT license
- BLS certification required
- 2 years of experience preferred, with experience in hospice setting (end of life care)
- Experience with interactive scheduler (EMR: Netsmart) is preferred
- No work history gaps over 90 days without explanation

Roles & Responsibilities
- Provide direct patient care and assist with activities of daily living
- Support patients and families during end-of-life care
- Document patient care accurately in electronic medical records
- Collaborate with interdisciplinary teams to ensure quality care

Compliance / Notes
Reliable transportation is required; public transportation is not available in this area. Vehicles should be equipped with all-season or snow tires during the months of November to April. Holiday time off cannot be guaranteed.
