Overview Registered Nurse – Home Health (Travel) Saint Johnsbury, Vermont, United States Start date: 08/10/2026 Contract: 12 weeks Weekly pay: $2,228 – $2,358 per week Guaranteed hours: 36 hours Shift: Days (Monday–Friday, with possible weekend shifts) Setting: Travel nurse assignment delivering professional nursing services in home health and hospice care, serving patients of all ages in Vermont Key Highlights Weekly pay range: $2,228 – $2,358 per week Guaranteed hours: 36 hours per week Contract duration: 12 weeks Shift: Days, Monday–Friday with possible weekend coverage Start date: 08/10/2026 Travel assignment in home health and hospice services in Vermont Responsibilities Provide a full range of professional nursing services to patients of all ages in the home setting, including admission, initial evaluation, and ongoing care planning Develop and adjust individualized care plans in collaboration with the patient, family, and interdisciplinary team Deliver direct patient care: monitor vitals, administer medications and treatments, manage accucheck and wound care, and respond to health changes Educate patients and families on disease management, safety, medications, symptom management, and care continuity Coordinate with home health professionals and paraprofessionals to ensure high-quality, coordinated care across the patient’s plan of care Document care provided in the charting system in a timely, accurate, and compliant manner and participate in care conferences as needed Requirements Active Vermont RN license Minimum 1 year of prior clinical experience Ability to provide nursing care in patients’ homes and work within a home health/hospice context Strong communication and teamwork skills; ability to collaborate with an interdisciplinary team Willingness to work a daytime schedule (M-F) with potential weekend shifts Eligibility for a travel assignment and ability to meet assignment start and duration requirements Benefits and Support Dedicated travel recruiter and assignment support throughout the engagement On-call or rapid-response clinical support to address questions and care needs Credentialing and onboarding assistance to facilitate a smooth start Ongoing scheduling coordination and assignment management Access to resources and guidance for successful remote documentation and patient handoffs General employment logistics support to help manage travel and housing needs (details provided during the interview process) Please note: Pay rates and guaranteed hours are estimates and may vary.
