PACT Clinical Pharmacy Specialist

Provides advanced care management within the Patient Aligned Care Team (primary care). Prescribes, monitors, and adjusts medications under a scope of practice. Obtains and evaluates medical and medication histories; orders and interprets labs. Designs individualized therapeutic plans including chronic disease management for: Diabetes, Hypertension, Hyperlipidemia, Heart failure, COPD/Asthma, Thyroid disorders, Osteoporosis, Smoking cessation. Performs risk assessments including suicide screening when indicated. Accepts urgent/same-day visits (polypharmacy review, hospital/ED discharge follow-ups). Utilizes multiple modalities (face-to-face, telehealth, secure messaging, telephone). Conducts medication reconciliation and verifies medication orders.

Reviews medication regimens for clinical appropriateness and safety. Monitors and reports adverse drug reactions (via VA ADERS). Utilizes population health dashboards to identify Veterans needing care management support and meet performance goals (SAIL, HEDIS). Provides consultative services and drug information to PACT providers. Manages consults, view alerts, CPRS functions, and documents timely notes. Supports coordination between inpatient and outpatient care.

Work Schedule: Monday through Friday varies may include some weekends and holidays
