Patient Care Coordinator

Job Purpose:- The Patient Care Coordinator at Crystal Finishing Systems is responsible for ensuring high-quality patient care by coordinating and facilitating communication between patients, healthcare providers, and administrative staff. This full-time role involves managing patient schedules, handling inquiries, and ensuring a seamless patient experience from admission to discharge.Key Responsibilities:- Coordinate patient appointments and manage scheduling to optimize clinic operations.- Serve as the primary point of contact for patients, addressing inquiries and providing information about services and procedures.- Collaborate with healthcare providers and administrative staff to ensure effective communication and continuity of care.- Maintain accurate patient records and update information in the healthcare management system.- Assist in the development and implementation of patient care plans and follow-up procedures.- Ensure compliance with healthcare regulations and company policies.- Participate in team meetings and contribute to the continuous improvement of patient care services.- Provide support for billing and insurance-related inquiries, ensuring patients understand their financial responsibilities.Required Education:- Bachelor's degree in Healthcare Administration, Nursing, or a related field is preferred.Required Experience:- Minimum of 2 years of experience in a healthcare setting, preferably in patient coordination or case management.- Experience working with electronic health records (EHR) systems.- Demonstrated experience in providing exceptional customer service in a healthcare environment.Required Skills and Abilities:- Strong organizational skills with the ability to manage multiple priorities and tasks efficiently.- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals.- Proficiency in Microsoft Office Suite (Word, Excel, Outlook) and other relevant software applications.- Ability to work independently and collaboratively within a team-oriented environment.- Strong problem-solving skills and the ability to make informed decisions quickly.- Knowledge of healthcare regulations and compliance standards.- Ability to maintain confidentiality and adhere to HIPAA regulations. #J-18808-Ljbffr
