Activity Coordinator Adult Day ProgramsGrace Community Care & Homes is developing its Adult Day Programs department and seeks an Activity Coordinator Adult Day Programs. The role will contribute to person-centered services, reliable operations and accurate documentation.Plan and deliver accessible activities based on participant interests and physical, cognitive and social needs; maintain calendars and participation records.Work collaboratively with department leadership, intake, credentialing, billing and quality teams. Maintain confidentiality, respect participant choice, communicate service gaps promptly, and follow role-specific policies and professional scope. Participate in orientation, competency review and quality improvement.This is a planned role. Start date and duties are contingent on applicable program approvals, funding, credentialing and staffing readiness. Services may begin only within the approved scope.New Jersey, with administrative coordination through Grace and on-site or community assignments as applicable. This is a part-time role. The final assignment and weekly schedule will be discussed during hiring. Clinical services begin only at approved locations and within the program's authorized scope.
