Licensed Practical Nurse- Home Health

Licensed Practical Nurse (LPN) Location: Harrisburg, PA 17111 (Dauphin and Cumberland County). BAYADA Home Health Care is looking for a compassionate and dedicated Full Time LPN to join our team in adult clients on a per visit basis in senior living communities throughout Dauphin, Cumberland, York and Lancaster counties. One year prior clinical experience as a licensed Practical Nurse is required. Performing assigned duties, including administration of medication, wound care, treatments and procedures. Monitoring clients' conditions and reporting changes to Clinical Manager and Client Services Manager. Following up with, executing, and properly documenting doctors' orders. Performing client assessments as necessary. Case management and coordination. Accurately documenting observations, interventions and evaluations using electronic medical records on a state‑of‑the‑art tablet. Qualifications Current license as a Licensed Practical Nurse in Pennsylvania. Minimum one year of recent, verifiable experience as an LPN. Graduation from an accredited nursing program, as indicated by transcript or diploma. Prior home care experience preferred but not required. Ability to work independently and manage time effectively. Strong interpersonal skills. Solid computer skills; prior experience with electronic medical records preferred. Ability to travel to cases as assigned.
