On-call hours are 5pm-8am, Monday-Friday, and all day Saturday and Sunday.As the On-Call Triage RN, you'll use your nursing expertise to assess patient needs, provide education and support, coordinate care, and guide timely interventions that allow the patient to remain safe and comfortable at home. Your expertise will directly influence patient outcomes, helping individuals and families feel supported during some of their most important moments of care.Your ImpactProvide telephonic triage support for home health patients and caregivers during evenings, nights, weekends, and holidaysAssess patient symptoms and clinical concerns, utilizing sound clinical judgment to determine appropriate interventionsProvide guidance regarding symptom management, medications, treatment plans, and home care instructionsEscalate urgent concerns and facilitate appropriate medical attention when necessaryCoordinate communication between patients, caregivers, physicians, and the home health care teamObtain, document, and communicate physician orders as neededAccurately document patient interactions, assessments, recommendations, and care coordination activities in the electronic medical recordEnsure continuity of care through detailed communication and handoff to daytime clinical staffAssist with after-hours patient inquiries and care coordination needsMaintain compliance with agency policies, regulatory requirements, and home health standards of practiceParticipate in quality improvement initiatives and ongoing clinical educationRequirementsCurrently licensed as a Registered Nurse (RN) in the State of Arizona1 year of home health experience strongly preferredPrevious triage, telehealth, on-call, or case management experienceKnowledge of Medicare home health regulations and standardsExperience supporting patients with chronic disease management, medication management, and complex care needsBenefitsMedical / Dental / Vision InsurancePaid Time Off401(k) matchingEmployee Assistance ProgramFSA / HSALife InsurancePet InsuranceEmployee Referral Program
