Key Responsibilities\n- \n
- Conduct comprehensive mental health assessments that evaluate the psychological impact of chronic medical conditions on patients’ mental health. \n
- Design and implement individualized treatment plans that address both the psychiatric and medical needs of patients, integrating therapeutic methods supported by the latest evidence. \n
- Facilitate both individual and group therapy sessions using tailored approaches to meet diverse patient needs, fostering a safe and encouraging therapeutic environment. \n
- Establish collaborative relationships with medical providers to enhance overall patient care and ensure a cohesive treatment plan. \n
- Provide crisis intervention services and immediate support during mental health emergencies, ensuring patient safety and appropriate follow-up. \n

- Maintain meticulous and confidential documentation of patient interactions and treatment plans, compliant with legal and ethical standards. \n

- Stay informed of new research and advancements in integrated mental health care to enhance clinical practice. \n
You will primarily work with patients dealing with co-occurring mental health and medical conditions, such as chronic pain syndromes, autoimmune disorders, and substance use disorders. Understanding the complexities of these interactions and addressing them through integrated care will be central to your practice, ensuring that our patients receive holistic support tailored to their unique circumstances.
Qualifications\n- \n
- Master's or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution. \n
- Current and valid licensure as an LCSW, LPCC, or LMFT in the state of California. \n
- Proven proficiency in recognizing and addressing mental health needs within the context of concurrent medical issues. \n
- Experience in integrated care settings and collaboration with medical professionals is essential. \n
- Exceptional interpersonal and communication skills, allowing for effective patient engagement and rapport building. \n
- Dedicated to maintaining professional boundaries, adhering to ethical guidelines, and safeguarding patient confidentiality. \n
- Familiarity with electronic health record (EHR) systems and telehealth software applications. \n
