Licensed Practical Nurse (LPN) - Outpatient Infusion Suite

Description Soleo Health is seeking a Licensed Practical Nurse (LPN) to support our Specialty Infusion patients in Norcross, GA . Join us in Simplifying Complex Care! Evaluate and educate patients/ caregivers to safely administer home infusion therapy. Assess the home environment to identify required interventions for safety and appropriateness. Develop patients’ Care Maps in collaboration with clinical team members and evaluate response to therapy during each visit. Discharge patients appropriately and complete discharge summaries within 7 days. Maintain caseload scheduling and communications, updating schedules weekly and monthly. Document patient visits to physicians or labs on the calendar. Coordinate care via phone or voicemail based on urgency. Document all communications promptly. Assist with vital sign monitoring, PICC dressing changes, PIV insertions, and other administrative tasks per state nursing laws. Schedule Full‑time, 40 hours per week, Monday – Friday, 8:30 a.m. – 5:00 p.m. Licensed LPN in the state of Georgia. IV Certified or willing to become IV Certified. Requirements Graduate of an accredited LPN or LVN nursing program. Minimum 1 year of nursing experience, with background in infusion therapy preferred. Current LPN license in good standing. Excellent customer service skills. IV Certification required. Proficiency with Microsoft Excel, Word, and Outlook.
