Social Work Services at the Mount Sinai Hospital
The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. We believe that total patient care must emphasize the physical, emotional, and social needs of each patient and their care partners. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907, the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education, professional development and extra-curricular opportunities.
The Mount Sinai Adolescent Health Center ("MSAHC" or the "Center") works to break down racial, social and economic barriers to health care and wellness for young people by providing vital services - high quality, comprehensive, integrated and confidential - for all who come to the Center or school based health facilities, at no cost to them. The MSAHC advances adolescent and young adult health as a national imperative by serving as a leading center of clinical care, specialized training, innovative research and advocacy.
The A28 Social Worker will provide social work services to at risk adolescents and their families in an integrated medical and mental health primary care setting at MSAHC. She/he/they will provide short term individual, family, and group psychotherapy, intense case management, crisis intervention, and health education as part of an interdisciplinary medical and mental health service model. A28 SW will consult with medical providers who are working with high-risk patients with both medical and mental health needs and conduct clinical assessments on patients referred by medical providers. These include providers within primary care and child and adolescent psychiatry. All A28 SW's will participate developing an innovative model to engage and retain adolescents who have a variety of psychosocial needs, particularly focusing on individuals with a history of trauma (e.g. sexual abuse, domestic violence, community violence, physical abuse.)

A demonstrated commitment to diversity, equity, inclusion and belonging
Skillful in assessing patient needs in relationship to health and psychosocial functioning and establishing intervention accordingly
Significant experience working with low-income, inner-city, HIV positive, at-risk adolescents
Experience in delivering therapeutic interventions, such as preventative and risk-reduction models that reflect developmentally appropriate methods and materials in working with adolescents and young adults (i.e. TF-CBT, CBT, MI, DBT, EMDR, CPT, IPT-A)
Experience in providing individual therapy and group work
Experience working with populations with complex trauma histories preferred
Experience working with LGBTQIA identified adolescents and young adults preferred
Ability to be flexible and use non traditional approaches in working with young adults preferred
Excellent collaborative, clinical and communication skills
Excellent organizational and documentation skills
Skillful in collaborating with external resources and coordinating appropriate referrals
Working knowledge of state regulations related to child abuse, neglect, and other urgent care issues
Bilingual (English/Spanish) preferred

Responsibilities
As part of an interdisciplinary team, the social worker will actively engage in the assessment and treatment planning of patients referred from medical providers in Article 28 setting while providing short-term therapy as needed. They will develop therapeutic alliance and deliver therapeutic interventions while consulting closely with medical providers and child and adolescent psychiatrist who are working with patients. The social worker in this role will be expected to develop effective treatments plans and track treatment progress using validated instruments. They will p articipate in an interdisciplinary effort that fosters a cohesive culture across staff with improved communication and accountability while collaborating with all practices to ensure integration of the MSAHC integrative model of care.
Additionally, the social worker will provide transgender and gender non-conforming (TGNC) youth with individual, and family psychotherapy, intensive case management, crisis intervention, and health education. A28 SW's will also provide assessments for gender dysphoria and hormone and surgery readiness if needed. They will be expected to provide counseling related to medical transition. The social worker will be charged with ensuring that the above functions are delivered within a culture that is youth-centric, high quality, integrated and collaborative as well as in accordance with Mount Sinai's policies and procedures, New York State regulations and guidelines, and funder expectations.

Compensation Statement
Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $88691 - $101670.78 Annually. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
