Job Description
Job Opportunity Position Overview This position involves providing home care and hospice coverage. Employees will be required to participate in an on-call rotation and are expected to work on holidays as necessary. Key Responsibilities
- Complete OASIS documentation in accordance with agency policies, as well as state and federal regulations.
- Utilize the nursing process to assess patient and caregiver needs to deliver quality care based on evidence-based practices.
- Develop and communicate an effective and realistic care plan for each patient, involving patients and caregivers in the process.
- Act as Case Manager for assigned patients, managing case loads and visit schedules to meet agency productivity standards.
- Implement the medical plan of care, nursing interventions, and procedures to ensure safe and quality care for patients.
- Stay updated on advanced nursing skills, including IV maintenance and complex wound care, ensuring infection control standards are upheld.
- Collaborate effectively with LPNs, delegating tasks as necessary and ensuring patient needs are met according to the plan of care.
- Evaluate the effectiveness of the care plan, identifying problems and monitoring patient responses to ensure high-quality care.
- Maintain accurate and current documentation of nursing care provided, integrating information for the healthcare team's ongoing quality of care.
- Follow agency protocols, including documentation processes and ensuring timely completion of required forms during patient visits.
- Educate patients and caregivers on treatment options and procedures, ensuring informed decision-making in accordance with regulations.
- Identify interdisciplinary needs for patients and communicate these needs to relevant healthcare professionals in a timely manner.
- Complete necessary agency, state, and federal forms accurately and efficiently while ensuring availability during visits.
- Manage visits efficiently, ensuring that supervisory visits are not combined with routine visits when appropriate.
- Assess patient needs for Home Care Aide (HCA) services, developing and overseeing plans of care accordingly.
- Report patient incidents, such as infections or injuries, to the Quality Improvement Specialist promptly.
- Coordinate safe patient discharges and transfers, ensuring all relevant paperwork and patient information is processed correctly.
- Coordinate patient care with various stakeholders, including physicians and team members, to support the recommended care plan.
- Participate in monthly peer review audits, providing and accepting constructive feedback professionally.
- Facilitate case conferences with management and team members to enhance communication and documentation of meetings.
- Advocate for patient care issues, ensuring that the type and quality of care provided are sensitive to the needs of each patient and family.
