Lead Care Manager - ECM Field Coordinator

BLEHEALTH is seeking a Lead Care Manager in the Metro Los Angeles area to coordinate care for chronically ill and high-risk members. This role partners with members, families, providers, and community resources to deliver Enhanced Care Management (ECM) services.
