Medication Technician / Certified Nursing Assistant (Med Tech/CNA)

Position Summary

The Med Tech/CNA is responsible for providing safe, compassionate, and dependable care to residents while assisting with activities of daily living and medication administration. This position supports residents in maintaining their highest level of independence, dignity, comfort, and quality of life.

The Med Tech/CNA is expected to follow resident service plans, medication orders, facility policies, state regulations, and established standards of care while communicating changes or concerns promptly to the appropriate supervisor.

Essential Duties and Responsibilities

Medication Administration

• Administer medications according to physician/provider orders, the Medication Administration Record (MAR), facility policy, and applicable state regulations.
• Complete medication administration documentation accurately and immediately after medications are administered.
• Follow the rights of medication administration and verify resident identity prior to administration.
• Follow medication pass schedules and established administration windows.
• Document and report refused, held, unavailable, missed, or otherwise not-administered medications according to facility policy.
• Complete required documentation for PRN medications, including the reason for administration and follow-up on effectiveness.
• Follow ordered medication parameters and document required vital signs or observations.
• Report medication errors, discrepancies, adverse reactions, or concerns immediately to the appropriate supervisor.
• Maintain medication carts, medication rooms, and medication storage areas in a clean, organized, secure, and compliant condition.
• Monitor medication supply and report medications requiring reorder or replacement.
• Properly document receipt of medications and follow facility procedures for discontinued or expired medications.
• Protect medication keys, access codes, resident information, and other confidential information.

Resident Care / CNA Responsibilities

• Assist residents with activities of daily living according to their individual service plans.
• Provide assistance with bathing, dressing, grooming, toileting, continence care, oral care, mobility, transfers, and other personal care needs.
• Complete scheduled toileting and resident checks as assigned.
• Assist residents with meals, hydration, and feeding when required.
• Assist with repositioning, ambulation, transfers, and mobility devices using appropriate safety techniques.
• Respond promptly to resident call lights, requests, and emergencies.
• Maintain resident rooms and personal areas in a clean, safe, and comfortable condition within the responsibilities of the care team.
• Encourage residents to participate in their care and maintain independence whenever safely possible.
• Treat every resident with dignity, privacy, patience, compassion, and respect.

Documentation and Communication

• Complete MAR, ADL, service-plan, progress-note, and other required documentation accurately and within required timeframes.
• Report changes in a resident's physical condition, mental status, behavior, mobility, appetite, skin condition, pain, or overall functioning.
• Immediately report falls, injuries, medication concerns, suspected abuse or neglect, changes of condition, and other significant events.
• Complete incident reports and other required documentation before the end of the shift when applicable.
• Communicate relevant resident information during shift change and with appropriate members of the care team.
• Never falsify, pre-chart, or document care or medications that were not provided.

Safety and Resident Rights

The Med Tech/CNA must follow infection-control practices, emergency procedures, fall-prevention measures, transfer techniques, and other facility safety requirements. Employees are required to protect resident confidentiality and comply with HIPAA requirements.

Any suspected abuse, neglect, exploitation, mistreatment, or unsafe condition must be immediately reported according to facility policy and applicable reporting requirements.

Residents must be treated respectfully at all times. Employees may not use intimidation, retaliation, humiliation, inappropriate language, or unnecessary force when providing care.

Shift Responsibilities

Employees are expected to arrive on time and ready to work their assigned shift. Before leaving, the Med Tech/CNA must ensure assigned documentation and responsibilities are completed and provide an appropriate handoff to the incoming staff.

Employees must communicate immediately with management when an assigned responsibility cannot be completed rather than leaving the task undocumented or unresolved.

Qualifications

• CNA certification or other required caregiver qualification as applicable to the position and state.
• Medication Technician certification/training or authorization as required by applicable state regulations.
• Current CPR/First Aid certification when required.
• Ability to safely administer medications within the employee's authorized scope.
• Ability to provide personal care and assistance with ADLs.
• Ability to understand medication orders, MAR documentation, service plans, and facility procedures.
• Strong communication, organization, and documentation skills.
• Ability to maintain confidentiality and professional boundaries.
• Must meet facility background screening, health, training, and competency requirements.

Physical Requirements

The employee must be able to perform the physical functions necessary to safely assist residents, including standing and walking for extended periods, bending, reaching, pushing, pulling, and assisting with resident transfers and mobility. Appropriate equipment and required assistance must be used when a resident's needs exceed what can safely be performed by one employee.

Performance Expectations

Successful Med Tech/CNAs demonstrate reliability, accurate documentation, medication safety, resident-centered care, teamwork, professional communication, and accountability. Staff are expected to ask for assistance whenever they are uncertain about an order, medication, resident condition, or care requirement.
