Your Role

As a Homecare Nurse, you will provide individualized, patient-centered care while working closely with patients and their families:

• Deliver skilled nursing care in accordance with physician orders and care plans
• Assess, monitor, and document patient conditions, identifying physical and psychosocial changes
• Administer medications, treatments, and interventions, evaluating effectiveness
• Develop, implement, and adjust care plans based on patient needs (RN scope)
• Collect and interpret clinical data to support high-quality outcomes
• Communicate changes in patient status to physicians and care teams
• Educate patients and caregivers to promote independence, safety, and wellness

Qualifications

• Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Massachusetts
• Current BLS/CPR certification
• Current TB test or chest X-ray
• Strong clinical skills, attention to detail, and a patient-first mindset
• One year of nursing experience preferred (new graduates encouraged to apply)
