Regional Nurse Consultant (RNC) - Assisted Living/Memory Care

We are seeking a Regional Nurse Consultant (RNC) for memory care and assisted living facilities. This role will oversee and manage clinical operations, regulatory compliance, and staff training across multiple communities. This position is responsible for driving clinical excellence, managing state survey readiness, auditing documentation, and provide mentorship to Health Services Directors to ensure high-quality, person-centered care.

Essential Duties and Job Responsibilities

Responsible for internal regulatory/compliance audits, surveys, and inspections to ensure adherence to requirements and track performance via key metrics. Monitor compliance with Oregon Administrative Rules (OAR), federal regulations, and company policies, preparing communities for annual state surveys and implementing plans of correction.

  • Responsible for quality improvement functions, and implement policies, procedures and processes that provide structure to Avamere's systems and facilities for regulatory and clinical standards compliance.
  • Provide training, education, monitoring and the necessary assistance to maintain the highest level of care to drive quality outcomes.
  • Conduct regular on-site audits of clinical documentation, medication management, infection control, and resident care records.
  • Provide ongoing training and mentoring to community-based nurses (Health Services Directors) and nursing staff on clinical best practices, assessment tools, and care planning.
  • Responsible for assuring all corporate procedures and systems are in place throughout the company and that the systems are in place to promote the highest level of care.
  • Develop and adhere to a proactive facility visits schedule based on facility risk and clinical goals.
  • Educate and assist clinical leaders in region to be survey ready at all times and ensure documentation is complete and consistent with legal requirements.
  • Review all surveys within the region for plans of correction and review weekly reports for each facility identifying areas of concern or priorities.
  • Provide reports and trends to the company and assist in developing plans to improve various areas of focus.
  • Educate, review and evaluate clinical practice and documentation integrity during onsite visits and during virtual review.
  • Perform and implement compliance programs including Mock surveys, system reviews and routine facility visits to ensure ongoing compliance with federal/state and Company policies.
  • Conduct and facilitate regional meetings and facilitate orientation for all newly hired Director of Health Services.

  • Must be able to innovate and be creative; ability to "think outside of the box" to solve problems.

Requirements and Qualifications

  • Must possess a current, unencumbered, active license to practice as a Registered Nurse in the state of Oregon
  • Must possess a nursing degree from an accredited college or
  • Must have a minimum of 3 years of experience as a Director of Health Services in Assisted Living/Memory Care facility; five (5) years of experience in Long Term Care and Skilled Nursing Facility settings.
  • Prior demonstrable experience developing, implementing, and monitoring systems that ensure clinical excellence, compliance with regulations, standards and policies of the company.
  • Expertise in Infection Control/Antimicrobial
  • Knowledge and understanding of Quality Measures and governmental programs to drive quality within Centers.
  • In-depth understanding of clinical and regulatory compliance requirements (Federal and State-OARs) for Assisted Living and Memory Care Facilities.
  • Experience in the principles of Quality Assurance and QAPI
  • Possess clinical and nursing knowledge necessary for the position and ability to effectively communicate that knowledge in order to accomplish the expected
  • Knowledge of emergency, and disaster processes
  • Demonstrative leadership qualities and a history of successfully integrating
  • Must have a valid driver's license and dependable vehicle for
  • Experience with Electronic Medical Records and computer documentation
  • Maintain confidentiality of all resident care information in accordance with HIPAA
