Responsibilities

• Provide direct nursing care to residents in a long-term care setting under supervision
• Administer medications and treatments as ordered; document administration and care activities
• Monitor residents’ vital signs, fluid intake/output, and ongoing health status; report changes to the supervising RN
• Assist with activities of daily living, mobility, and fall prevention; promote safety and comfort
• Collaborate with the interdisciplinary team to implement and adjust individualized care plans
• Maintain accurate documentation in the electronic medical record and uphold infection control and safety standards

Requirements

• Active Licensed Practical Nurse license
• BLS/CPR certification
• Ability to work in a long-term care environment and fulfill a 3x12-hour shift schedule
• Strong communication and documentation skills; ability to work within an EMR system
• Professionalism, reliability, and teamwork in a healthcare setting
• Willingness to participate in credentialing processes and adhere to facility policies
