Community Health Navigator - Field Outreach & Resources
AbsoluteCARE Medical Center & Pharmacy
AbsoluteCare Medical Center & Pharmacy in Cleveland, OH seeks a Community Health Worker to connect members with resources, address social needs, and support enrollment in services. This role involves visiting members at home and in the community to assess needs and build trust. You will document outreach in Jiva, eCW, and CRM systems, and collaborate with the care team to meet engagement and enrollment targets.
