Outpatient Family Medicine Physician Community Care Leader

St. Charles Health System

St. Charles Family Care seeks a Family Medicine Physician to deliver comprehensive outpatient care, manage treatment plans, and coordinate with the care team to achieve high-quality outcomes.

The physician will work 40 hours weekly with 36 hours of direct patient contact, participate in clinic call rotations, and perform administrative duties including charting and referrals within SCHS policies.
