Remote Geriatric Care Coordinator

Pine Park Health Management Inc. is seeking a Certified Medical Assistant to serve as the Remote Primary Care Coordinator. You will be the operational and relational backbone of our geriatric primary care team, coordinating care for older adults across AZ, CA, and NV from home. In this fully remote role, you will triage urgent needs, maintain accurate EMR documentation, schedule follow-ups, onboard new patients within 24 hours, and collaborate with families, facility staff, and providers to #J-18808-Ljbffr
