Program Manager

245D Program Manager – Intake & Client Services $60,000–$65,000 per year | Full-Time | Hybrid | Minnesota Hearts in Homes is looking for an experienced Minnesota 245D professional to take ownership of our intake and client-services department. This is not an entry-level intake position. We are looking for someone who already understands 245D, knows how to work with county case managers and families, and can confidently move a referral from initial contact through intake and a successful start of services. This person will have the opportunity to run the intake function, strengthen our systems, build referral relationships, and eventually help build and lead a larger intake team as we continue to grow. Applicants must currently reside in Minnesota and be able to travel within our service area for in-person intakes and meetings as needed. This is not a fully remote position. What You’ll Own Referrals, screenings, intakes, admissions, and service-start coordination Required 245D intake documentation, authorizations, support plans, signatures, and follow-up Communication with members, families, guardians, county case managers, and referral partners Coordination of caregiver matches, meet-and-greets, and successful service starts Ongoing follow-up to make sure new services are going well and concerns are addressed early Accurate, organized, audit-ready member files and required documentation Building and maintaining strong relationships with Minnesota case managers and referral sources Improving intake processes and helping develop the department as the company grows What We’re Looking For 3+ years of experience in Minnesota 245D, disability services, or a closely related human-services setting Hands-on experience with 245D intakes, service starts, documentation, and service coordination Experience communicating directly with county case managers, families, guardians, and support teams Strong understanding of Minnesota 245D requirements and disability-service systems Existing relationships with Minnesota case managers and/or referral sources, or demonstrated experience building those relationships Strong organization, communication, judgment, and follow-through Comfortable working independently and taking ownership without needing constant direction Proactive and solutions-oriented—you notice problems, address them, and follow through Comfortable learning technology and working in electronic systems Interested in helping build and eventually lead a strong intake and client-services team About Hearts in Homes Hearts in Homes is a growing Minnesota 245D provider.
