Physical Therapist Assistant

Provide skilled physical therapy services under the supervision and direction of a licensed Physical Therapist. Implement treatment plans to improve residents mobility, strength, balance, and functional ability. Deliver direct therapy services, including therapeutic exercises, gait training, and balance activities. Observe and report residents progress and responses to therapy to supervising PT. Assist with resident assessments, documentation, and care planning. Educate residents and caregivers on exercises and safety techniques for continued progress outside therapy sessions. Maintain accurate and timely documentation in electronic medical records. Ensure proper use and care of therapy equipment and assistive devices. Participate in team meetings and collaborate with interdisciplinary staff to support coordinated care. Adhere to state, federal, and facility regulations and maintain professional and ethical standards. Work as part of a rehabilitation team in a skilled nursing facility/long-term care environment.
