Why you’ll love working here: Opportunities for Year-End Bonus Weekly Pay Paid Time Off (PTO) Accrual Starts Day 1, PTO can be used after 60 days Flexible Spending Accounts for healthcare and dependent care 401k retirement savings program What you’ll do: A professional staff person is assigned duties as the QIDP for selected individuals we support, having the following responsibilities (483.430/W159) to: 1) Orchestrate all facets of the Active Treatment effort, including the IDT creation of relevant IPPs tailored to meet the needs of the individuals we support; 2) Effectively coordinate internal and external program services and supports to facilitate the acquisition of individual skills and adaptive behaviors; and 3) Promote competent interactions of residential and program staff with individuals in program implementation and behavior support. Serve as QIDP for select individuals supported at Cedar Lake Lodge. Review all reports and documentation to include but not limited to: medical reports, programming reports, and behavioral reports. Review IPP summary and IDT meeting notes to ensure the implementation of recommendations and provision of services. Become and remain very familiar with each assigned individual supported at Cedar Lake Lodge, following, identifying, and addressing trends that both positively and negatively impact each individual. Review monthly QIDP summaries (provided by the QIDP Clerks) to ensure timeliness of documentation, appropriateness of goals, and procedure and progress attained. Review plans of care for individuals we support for appropriateness and follow-up on recommendations. Maintain accurate documentation in the IPP to reflect any changes in family status and/or guardianship. Maintain contact with individuals' families and guardians to keep them up to date on individuals’ status. Review incident reports for each assigned individual we support at Cedar Lake Lodge. Make prompt referrals as appropriate to staff for program revision or additional interventions as needed. Attend Interdisciplinary Team reviews for the individuals we support and document attendance. Advocate for the individuals we support and assist staff in advocating for the wants and needs of the individuals. Maintain accurate records on the needs and progress of the individuals he/she supports. Make visits to all daily program areas in-house and suites to observe the individuals he/she supports. Make monthly visits to off-site programs (if applicable) to observe the individuals he/she supports to ensure the individuals’ IPPs are being followed and to follow up on concerns of the individual and staff as needed. Document all individual rights restrictions and obtain approval for these restrictions. Perform other duties as assigned. What sets you apart: Bachelor’s degree in Human Services field and/or RN required. A minimum of 1-year experience working with individuals with intellectual and developmental disabilities is required. Must have a valid Kentucky or Indiana Driver’s License and a clean driving record.
