This helps us get candidates presented quicker as we all know these orders open/close quick! This form needs to be 100% accurate and have all requests noted in the highlights section. Start Date should be on or very quickly after the Order Start Date listed. All RTO should be listed on the cover page, must be less than 7 days. RTO will not be approved during the first week of the assignment. All facilities the traveler is open to working in should be listed on the submission. If interested in "any" location, please include what states (I.E. Any AZ, Any CO, Any state, etc.). All shifts the traveler is open to working on should be listed on the submission. All departments the traveler is open to working must be checked on the cover page. All candidates submitted should be preclosed and ready to accept. Please let us know if the traveler withdraws or is no longer available.

SUBMISSION REQUIREMENTS

• Minimum 24 months of current CST experience - REQUIRED
• Certifications must be American Heart Association OR client and active upon submission - MUST BE INCLUDED IN SUBMISSION - REQUIRED
• No Nursys Hits - REQUIRED
• Banner will not approve RTO within the first week or holidays. Can not exceed 7 days.
