Registered Nurse - Home Health

Job Description A day in the life: manage an assigned caseload from admission through discharge using a framework of assessment, planning, coordination, and evaluation. Responsibilities include completing comprehensive assessments (including OASIS when required), developing and updating individualized plans of care, and providing skilled nursing services across preventative, rehabilitative, palliative, and end‑of‑life needs. Educates and counsels patients and families on disease processes, symptom management, and self‑care, and coordinates care with interdisciplinary team members such as Home Health Aides, Therapies, Social Work, Chaplaincy, and Bereavement Services. This position also supervises LPNs and paraprofessionals, communicates changes in patient condition to physicians and team members, secures necessary equipment and supplies, and maintains thorough, timely documentation within the electronic medical record. Qualifications Licensed RN in the State of Michigan Experience in acute care, home care, or hospice preferred Proven health assessment, organizational, and interpersonal skills with the ability to make independent judgments directing patient care. Able to meet Home Health and Medicare standards through criminal background check and fingerprinting. Possess current Michigan motor vehicle license, ability to drive a car and an insured vehicle capable of transport to patient homes or other offices in various weather conditions.
