All positions will be considered, fulltime, PRN, and part-time. Serious inquiries only and those that are in pursuit of a *Brave*new chapter.**Duties*** Conduct thorough assessments of patients' health conditions and needs.* Develop and implement individualized care plans based on patient assessments.* Administer medications and treatments as prescribed, including injections and tube feeding.* Monitor vital signs and document changes in patients' conditions.* Provide specialized care for patients with specific needs such as dementia care, or ostomy care.* Educate patients and their families about health management and disease prevention.* Collaborate with physicians and other healthcare professionals to coordinate patient care.* Maintain accurate medical records in compliance with ICD-10 coding standards and Medicare guidelines.## Qualifications**Skills** The successful candidate will demonstrate the following skills:* Proficient in dementia care, wound care, and chronic disease management techniques and strategies.* Proficient in post-operative and post-acute inpatient care.* Experience in administering injections safely and effectively.* Knowledge of tube feeding procedures.* Ability to assess and monitor vital signs accurately.* Familiarity with ostomy care practices.We encourage tenured and newer clinicians to explore what makes Bravery different in skilled home care delivery.Job Types: Full-time, PRNBenefits: Full Time* 401(k)* Dental insurance* Health insurance* Paid time off* Vision insuranceWork Location: In person #J-18808-Ljbffr Bravery Home Healthcare
