MDS Assessor

Park Nursing Home is hiring an MDS Assessor to work in-person at its Skilled Nursing Facility in Rockaway Park, NY. We are a 196 bed facility located right by the boardwalk at Rockaway Beach. We have a beautiful view of the Atlantic Ocean and we are accessible to the subway, water ferry and buses. Required: Registered Nurse (RN) License
DUTIES:
Completing accurate assessments, MDS & care plans as assigned Initiating care plans and supporting activities as assigned Creating and distributing monthly care plan calendars in a timely fashion Maintaining & updating all care plans and assessments as required Monitoring & auditing clinical records, ensuring accuracy & timeliness Informing DON of persistent issues related to non-compliant documentation Protecting the confidentiality of Resident & Facility information at all timesREQUIREMENTS:
