Job Description

This position requires providing high-quality healthcare services in a post-acute setting. Candidates will be responsible for ensuring patients receive appropriate care and support during their recovery process.

Responsibilities

• Deliver patient care according to established protocols.
• Monitor and document patient progress.
• Collaborate with healthcare team members to develop and implement care plans.
• Assist patients with daily activities and rehabilitation exercises.
