Registered Nurse Case Manager Home Health/ Full-Time Location:Attleboro, MA Pay:$93,000-$104,000 Annual Shift Options:M-F 8a-5p Full Job Description Position Overview Energy Employee Services, LLC: Helping eligible energy workers obtain the compassionate healthcare they deserve! We are seeking a compassionate Registered Nurse Case Manager to join our team. As a member of the home health team, you will be responsible for managing the home care of terminally ill or elderly patients. The ideal candidate will also have strong communication skills and a committed dedication to providing patients and families with accessible, comprehensive care. Responsibilities Thoroughly assess patients in their homes to ensure best patient care. Oversee the patients Plan of Care (POC) as provided by the physician Guide and mentor Field Staff to ensure Plan of Care is being followed accurately Work closely with staff and patients provider to ensure best outcomes for the patient. Provide clear medication education and medication management Assist with Activities for Daily Living (ADLS) to help with the patients quality of life. Must be willing to participate in the On-call rotation as assigned, to ensure best patient care and outcomes! Timely and accurate documentation of patient status and condition. Other tasks as assigned. Qualifications Must be willing to submit to a background check Must have a valid driver license, reliable transportation and auto insurance Must have an unencumbered RN License in the state you are applying to.
