A rewarding opportunity is available for a Registered Nurse with home health expertise to make a difference in patient lives through compassionate in-home care. This full-time, 13-week assignment provides a consistent Monday through Friday schedule, working 8:00 AM 5:00 PM, and offers a manageable caseload of approximately 35 patient visits per day within a defined territory.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing care in patient homes, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress and promptly communicate any health changes to the care team and physicians.
• Educate patients and their families about disease processes, care plans, medications, and preventive health measures.
• Ensure timely and accurate documentation using Home Care Home Base (HCHB).
• Coordinate services with physical therapists, occupational therapists, medical social workers, and home health aides.
• Attend case conferences, staff meetings, and participate in ongoing education.
• Adhere to all regulatory requirements and agency standards.

Qualifications:

• Active RN license in the state of practice, in good standing.
• At least 1 year of recent home health nursing experience (field experience preferred).
• Proficiency with Home Care Home Base (HCHB) and OASIS required.
• Reliable transportation, valid drivers license, and auto insurance.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
