We are seeking a compassionate and organized Social Service Designee to join our dynamic team in Conneaut, Ohio. As a Social Service Designee at Lake Pointe Rehabilitation & Nursing Center, you will play a vital role in providing exceptional care to our short-term residents. Our facility is well-known for its high-quality care, and we are committed to helping our residents reach their goals.
About the Role:
As a Social Service Designee, your primary responsibilities will include:
Conducting social service assessments in Point Click Care and maintaining accurate records
Coordinating ancillary services for all residents, including discharge planning and equipment ordering
Identifying and addressing social service concerns that may arise for each resident
Collaborating with the administrator to ensure effective communication and resolution of concerns
Maintaining organization and prioritizing tasks to meet deadlines
Requirements:
High school diploma or equivalent required
Ability to work independently and as part of a team
Excellent communication and documentation skills
Strong organizational and problem-solving skills
Ability to maintain confidentiality and handle sensitive information
