• Conduct thorough assessments of new residents to identify their needs, goals, and preferences
• Develop individualized care plans that align with each resident's unique needs and goals
• Collaborate with interdisciplinary teams to implement and monitor care plans
• Provide ongoing assessment and management of resident conditions, including chronic illnesses and rehabilitation needs
• Communicate effectively with residents, families, and healthcare professionals to ensure open and transparent communication
• Maintain accurate and detailed records of assessments, care plans, and resident progress
• Participate in quality improvement initiatives to ensure continuous quality improvement and excellence in care
