Key Highlights Weekly pay: $2,264–$2,365 per week Contract length: 8 weeks Start date: 08/03/2026 Shift: Day shift, 40 hours per week Setting: Home health within a hospital‑system EPIC transition project Travel assignment through Healthforce Healthcare LLC Responsibilities Deliver physical therapy services to patients in a home‑based, transitional care context Conduct initial evaluations, develop and implement individualized plans of care, and adjust treatment as needed Administer evidence‑based interventions (therapeutic exercise, gait and balance training, ROM, strengthening) and monitor progress Document patient assessments, treatments, and outcomes in EPIC and Cerner with accuracy and timeliness Collaborate with care teams, physicians, and family/caregivers to optimize safety and functional independence Provide patient and caregiver education on home exercise programs, mobility, fall prevention, and safety strategies Ensure compliance with regulatory and payer guidelines and maintain high standards of patient safety and confidentiality Requirements Recent experience using EPIC and Cerner systems Experience delivering PT services in a home health or equivalent setting Availability for a day shift, 40 hours per week, on an 8‑week assignment Ability to work in a home‑based clinical environment and participate in a system‑wide EPIC transition Strong communication and collaboration skills to coordinate with the care team and patients Benefits and Support Assignment support from Healthforce Healthcare LLC Access to ongoing recruiter and travel support throughout the assignment Clear onboarding for EPIC/Cerner transition project Professional support to facilitate successful completion of the assignment #J-18808-Ljbffr
