Registered Nurse, Case Manager SNF - geriatric (discharge planning, resources) SNF guidelines/experience preferred Customer service - interpersonal skills Case management experience required BSN preferred SOME TRAVEL REQUIRED Home base Manteca, will travel to Stockton mainly, sometimes to Modesto/CVA 1-2 days at assigned facility, other 3 days in office
