We are hiring a Physical Therapist (PT) for a Home Health position providing skilled care to patients in their homes in the Bushkill, PA area. This position is eligible for $20,000 total bonus potential, including a $10,000 first year success bonus and a $10,000 sign‑on bonus paid after 30 days of employment. To receive these bonuses, you must be in good standing and meet performance expectations, and remain in the role for which you were hired. The bonus opportunities extend well beyond your first year. What You’ll Do Deliver skilled, patient-centric care in a home health setting, collaborating with an interdisciplinary team that truly supports you. Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence. Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home. Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation. Work independently, but never alone. Travel to patient homes within assigned territory. PT Qualifications Active Physical Therapist, PT license in the state you work. One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care.
