Our mission statement definitely speaks to what La Clinica de Familia stands for, which is to empower and enrich families, individuals, and communities by providing quality medical, dental, behavioral health and educational service for people of all cultures. up to $24.51 Non-Exempt Job Summary The CCBHC Patient Specialist provides a full range of professional nursing services, including assessment, planning, implementation, evaluation, and coordination of patient care in accordance with individualized, team-based Comprehensive Care Plans. Under the direct supervision of the CCBHC Clinical Team Lead, the CCBHC Patient Specialist plays a critical role in supporting individuals as they navigate behavioral health, physical health, and social service systems. The CCBHC Patient Specialist will work as part of a multidisciplinary CCBHC team to improve health outcomes, reduce barriers to care, and promote recovery and wellness. This role addresses social determinants of health, identifies barriers and provides community-based resources and services. Utilize informatics - communicate, manage knowledge, mitigate errors, and support decision making using information technology. Nursing or Social work degree preferred. Minimum one (1) years’ experience in clinical care setting Community Health Worker Certification or be able to obtain within first three months to six months of employment.
