LPN Continuing Care

Job Title: LPN Continuing Care Profession: Nursing Specialty: Agency Staff LPN Duration: 13 weeks Shift: 2x3 nights Hours per Shift: 12 hours Experience: Minimum 2 years of experience License: LPN License Certifications: BLS Must-Have: - Rehab experience - Familiarity with EPIC - Experience with trachs, admissions, feeding tubes, and neuro patients - Previous work in a skilled nursing facility Description: As a continuing care LPN, providing top-notch patient care is paramount. You will be responsible for assessing, planning, implementing, and evaluating patient care under the direction of an RN, mid-level provider, or physician. You will play a vital role in our healthcare team. Your work will make a real difference in the lives of patients and their families. You will collect and monitor assessment data. Report physiological, psychological, sociocultural, and spiritual needs of patients and families in coordination with the healthcare team. You will provide direct patient care consistent with policies, procedures, and unit-based routines. Inform patients and families about treatments, medications, and equipment being utilized. Document care provided in accordance with guidelines. You will greet and communicate with patients and families with care and respect. Collaborate with other members of the healthcare team to ensure continuity of care and service coordination. Clean the work area and communicate any equipment malfunctions to the appropriate personnel. Document patient chargeable items according to standards. Participate in selected quality monitoring activities under the direction of the healthcare team. Demonstrate a high standard of moral and ethical behavior. Recognize appropriate and ethical behaviors and identify resources for resolution as needed. Advocate for privacy, confidentiality, and security of patient and organizational data. Actively participate in safety initiatives and complete all position and unit safety-related competencies and requirements promptly. Perform other duties as assigned.
