Nurse Practitioner/ APP | Home Health | Philadelphia

Job Description We’re a Value‑Based Care group on a mission to rebuild healthcare as it should be, providing personalized primary care for older adults on Medicare, with the goal of keeping patients healthy and living life to the fullest. Our innovative care model is centered right in our patients communities and focused on the quality of care over the volume of services. Salary Range for both is 105,000-135,000 (Experienced provider with bonus can make $150‑$160k). Annual clinical performance bonus (last year NPs 15k annually and more seasoned with 5+ years exp. Available for 25k bonus). Reimbursement for mileage and toll fees. Role: Home Health Care Each NP or APP will be supported by a care team traveling with them. NP's Care Team: Their own Medical Assistant, Scribe & a designated Social Worker. Average 6-7 visits a day. Will be supported by the Medical Director who will serve as NP/APP's mentor. NP/APP does same work up as they would in a clinical visit. Required Qualifications Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate. National certification in at least one of the following specialties: Family Nurse Practitioner Adult‑Gerontology Primary Care Nurse Practitioner Adult Nurse Practitioner Gerontological Nurse Practitioner Active, non‑probationary state Nurse Practitioner license. Active DEA license. US Work Authorization. Will hire a newer NP and provide mentorship for the role. #J-18808-Ljbffr 6AM City, LLC
