Physical Therapist (PT) – Home Health Visits

Full-Time | Hudson County, NJ

$10,000 Sign-On Bonus for Full-Time!

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) to join our Hudson County Visits team.

This position provides one-on-one home health visits for adult and geriatric clients throughout Hudson County, NJ , with an immediate opening in the southern Hudson County (Jersey City/Bayonne) area.

Responsibilities

• Perform skilled physical therapy visits throughout designated territories.
• Complete comprehensive physical therapy evaluations and ongoing reassessments.
• Assess mobility, strength, balance, gait, transfers, endurance, and overall functional status.
• Develop and implement individualized treatment plans using evidence-based interventions.
• Provide therapeutic exercise, gait training, balance training, manual therapy techniques, and patient education.
• Collaborate with nurses, physicians, and interdisciplinary team members to coordinate exceptional patient care.
• Educate patients and caregivers on exercises, safety strategies, fall prevention, and disease management.
• Complete timely and accurate documentation using BAYADA's electronic medical record (EMR) system.

Qualifications

• Current New Jersey Physical Therapist (PT) license.
• Graduate of an accredited Physical Therapy program.
• Home health experience preferred, but not required.
• Strong communication, organizational, and clinical assessment skills.
• Ability to work independently and effectively manage a flexible schedule.
• Experience with electronic medical records (EMR) preferred.
• Reliable transportation and ability to travel throughout the assigned territory.

Additional Information:

Base Pay, depending upon qualifications, status, and subject to negotiation:

$77.00-$83.00 per point ($120,120-$129,480 annual earning potential)

$10,000 Sign on Bonus! for full time PT positions.
