Performs plan of care interventions, including specimen collection and wound care. Demonstrates proper infection control practice and adheresto Infection Control Policies and Procedures and OSHA standards. Adheres to medication pass protocol and meets regulatory guidelines. Evaluates nursing care provided by continual review of the effectiveness of the nursing care plan. Collaborates with supervising nurse to make appropriate revisions to achieve the desired outcomes. Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.Works as an advocate for the physical and emotional well-being of the patient. Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families. Collects and documents patient information as needed (including weightsand vital signs). Provides patient and family education in regard to the hospitalization/admission, medications, and/or in preparation for discharge. Upon discharge, provides appropriate discharge instructions to include follow-up and referral needs. Provides accurate and timely hand-off communication. Serves as clinical and administrative resource to staff, residents, and families regarding nursing services and procedures, aging processes, medications, and outside medical services. Supervises CNAs. Job Requirements Education/Experience: Qualified candidates will have graduated from an approved Licensed Practical Nurse program Must possess a current, unencumbered license to practice as a LPN in this state. Licensure/Certifications: Current LPN licensed in good standing by their State Board ofNursing. Current BLS.
