Care Coordinator

Treatment Alternatives for Stronger Communities

Division Overview Adult Criminal Justice and Treatment Services (ACJS) supports clients who may have struggles related to a substance use disorder. ACJS provides opportunities for clients to rebuild their lives so they can become healthy and self‑sufficient. ACJS’ work includes screening and assessments for substance use disorders, placing clients into treatment programs, monitoring and reporting on progress, and providing support including insurance enrollment, obtaining a state ID, and finding employment resources. Position Summary The Collaborative Bridges team is an innovative care coordination pilot program to connect individuals and families with substance abuse and mental health needs upon their discharge from hospitals and provide supportive care coordination services until linked to long‑term community providers or client needs are resolved. The position will educate and guide patients on the critical role of primary care providers in maintaining their overall health and well‑being, emphasize preventive care, manage chronic conditions, and coordinate specialized treatments when necessary. Encourage patients to establish a relationship with a primary care provider within their community to ensure continuity of care, timely medical interventions, and a trusted source for health‑related guidance. Provide resources and support to help patients navigate the healthcare system, select an appropriate provider, and understand the value of regular check‑ups in promoting long‑term wellness. Essential Duties and Responsibilities Lead initiatives aimed at improving healthcare outcomes and reducing health disparities for justice‑impacted populations. Increase access to preventive, primary, and specialty care within the community, addressing health‑related social needs that impact access and overall health outcomes including chronic health conditions. Enhance care integration, efficiency, and coordination across various provider types and care levels. Commit to health equity, strategic program development, and collaboration with healthcare and community partners to drive meaningful change. Partner with Collaborative Bridges staff to provide clients with comprehensive medical care in addition to medication‑assisted recovery or other evidence‑based programs for their substance use disorder. Provide referrals and linkages to clients for identified needs and coordinate outreach services to community‑based social‑service agencies addressing social determinants of health such as employment, housing, food security, and transportation. Respect the diverse cultures of the people served and provide culturally appropriate, competent, and individualized treatment based on the client’s age, gender, race, ethnicity, and culture. Engage and educate clients for participation in treatment and other needed resources for all social‑determinant of health needs. Develop and maintain relationships with community‑based programs and attend linkage appointments when necessary and appropriate. Conduct outreach and follow up with clients, families, and treatment providers. Ideal Candidate High school diploma or GED equivalent. Valid driver’s license. CADC certification within two years of employment (may be required based on contractual obligations). Ability to pass an IDOC background check. Proficiency in Microsoft Office suite. Experience in group facilitation. Knowledge of current ASAM placement criteria, DSM‑V as it relates to substance‑related disorders, and the Code of Ethics for substance abuse professionals, or ability to acquire such knowledge. Comfort working in a correctional setting and with persons convicted of serious violent offenses. Flexibility in thinking and approaches to leading group discussions. Good verbal, written, and interpersonal communication skills. Good time‑management skills with a proven ability to meet deadlines.
