Outpatient/Inpatient Nurse Case Manager
This is a 13-week contract position.
Monday to Friday, 8 am - 5 pm & Weekend rotation with call.
The Outpatient/Inpatient Nurse Case Manager serves as a supportive contact for identified high-risk patients, focusing on specialty clinics requiring specific case management attention. This role ensures quality and cost-effective healthcare by facilitating care transitions from admission through discharge, ensuring continuity of care, and managing referrals to continuing care agencies. The case manager will interact with culturally diverse patient populations, addressing multiple barriers to self-sufficiency, such as illiteracy, domestic violence, and mental health issues.
Responsibilities:
- Assess essential health and social needs for patients.
- Collaborate with an interdisciplinary team to evaluate care plans and monitor progress.
- Coordinate care for patients with chronic pain and frequent utilization issues.
- Evaluate patients in the ED for medical necessity and care coordination needs.
- Propose and implement outpatient plans for discharge with physician approval.
- Help individuals with mental health issues access navigation resources.
Required Experience / Certifications / Licensure:
- Education: Associate's or Bachelor's Degree in Nursing.
- Must hold a valid, current, and unrestricted Professional Nursing License from any U.S. state or territory.
- Current Basic Life Support (BLS) certification is required.
- Three (3) years of clinical nursing experience with a minimum of one (1) year in an Outpatient/Inpatient setting.
- Demonstrated knowledge of case management and utilization review processes.
