Nurse Team Leader - Home Care

Compensation

Salary Range: $125,000 - $135,000 per year

Bonus / Commission (if applicable):
This role is eligible for a sign-on bonus or loan assistance of $15,000, paid over 3 years.

Location: Bronx, NY
Field-Based / Onsite

Position Summary
As a Nurse Team Leader in Home Care, you will provide clinical oversight and leadership to a team of field nurses while ensuring high-quality, patient-centered care for individuals in the community. This role focuses on managing care plans, mentoring staff, and coordinating services with interdisciplinary teams and families. You will play a key role in maintaining clinical excellence and supporting the delivery of home health services.

Responsibilities

• Shift details: Day shift
• Hours per week: 40

? Key Duties:

• Oversee and manage individualized patient care plans to ensure quality and continuity of care
• Supervise, mentor, and support home care nurses, including onboarding new team members
• Coordinate with interdisciplinary teams, patients, and families for comprehensive care delivery
• Document care activities and maintain accurate records using EMR systems
• Serve as a clinical leader in the field, providing guidance and ensuring compliance with home care standards

Requirements

? Education

• Required: Associate's Degree in Nursing
• Preferred (if applicable): Bachelor's Degree in Nursing (BSN)

? Licenses & Certifications

• Required: Active New York State Registered Nurse (RN) license

? Skills:

• Strong clinical expertise in home health and regulatory standards
• Leadership and supervisory skills with ability to mentor staff
• Proficiency in EMR systems and Microsoft Office
• Excellent problem-solving, communication, and organizational skills
