In-Home Health Advocate & Care Coordinator
Jobtailor
Jobtailor is seeking a patient-facing Care Coordinator to engage with patients in-home, support virtual visits, collect vitals, and help with screenings. You will build trust in communities, document in the EHR, and coordinate with the care team. The role requires CMA or CHW certification (or equivalent healthcare experience) and may favor bilingual candidates (Spanish/Arabic).
