Job Summary Provides physical therapy services to patients with impairments, functional limitations, disabilities, or changes in physical function resulting from injury, disease or other causes. Actively collaborates with the health care/educational team to meet patient care, outcomes and system improvement objectives. Key Responsibilities Analyze comprehensive information from formal and informal assessment to determine a rehabilitation treatment diagnosis. Collaborate with stakeholders to establish the plan of care, coordinate and implement care delivery while anticipating patient care needs across the rehabilitation continuum. Apply appropriate, evidence‑based interventions to ensure effective outcomes and re‑evaluate performance to assess patient’s progress toward goals. Employ strategies to promote successful return to home, community, and school/vocational environments. The responsibilities listed are a general overview and additional duties may be assigned. Technical Capabilities Treatment Planning & Goal Setting (Novice): Develop and document functional, measurable short‑ and long‑term goals and incorporate treatment interventions focused on meeting objective goals. Rehabilitation Therapist Patient Evaluation (Novice): Evaluate patients with physical deficits, administer and score standardized tests, select appropriate measures, perform functional assessment, interpret results, and communicate findings to the patient, family, physician and team. Treatment Implementation (Novice): Provide treatment consistent with physician prescription and established plan of care, modify plan as needed, supervise and direct certified athletic trainers, physical therapist/occupational therapist assistants, technicians and volunteers and students, select correct protocol and utilize evidence‑based treatments. Rehab Therapist Patient Documentation (Novice): Document in the appropriate templates, use approved abbreviations, and meet quality and timeliness standards. Physical/Occupational Therapist Patient Education (Novice): Communicate functional impact of assessment findings, treatment interventions, diagnosis and disease process, consider communication needs, and may supervise or train students from professional programs. Position Qualifications Certification: Licensed Physical Therapist, State of Tennessee. Experience: Minimum 1 year of relevant work experience. Education: Bachelor’s degree.
