Lead Care Manager, ECM — Field Health Navigator
BLEHEALTH
BLEHEALTH is seeking a Lead Care Manager in the Metro Los Angeles area to coordinate care for chronically ill members. This role works with members, families, providers, and community resources to deliver Enhanced Care Management (ECM) services in person. You will cover field visits, care planning, and collaboration with hospitals and clinics, ensuring timely access to services, documentation accuracy, and compliance with HIPAA and MCP guidelines. #J-18808-Ljbffr BLEHEALTH
